• Email Address

• Phone Number

If your team is not yet complete, please enter “N/A” for the name and email and “000-000-0000” for the phone number of any remaining players. Any changes to the team must be submitted by Wednesday, September 17th.

Incomplete forms may delay the processing of your registration.
